About Us
Jewish Family Services of Northeastern NY (JFS) is looking to recruit a Nurse (LPN or RN) for the Albany NNORC (Neighborhood Naturally Occurring Retirement Community).

The Albany Neighborhood Naturally Occurring Retirement Community (NNORC) is a unique, three-square mile area in Albany, NY that has a high concentration of people 60+. The staff of the NNORC program coordinates services and community resources that help older adults maintain their quality of life and independence, access services, and remain connected to their neighborhoods. By supporting aging in place, NNORC not only helps older adults but contributes to the diversity and stability of their neighborhoods. 

The is a wonderful and unique opportunity to work closely with older adults in the role of advocate, evaluator, health educator, and health promoter while also offering and providing health monitoring services. The ideal candidate will be able to foster relationships with older adults living in this region (near St. Peter’s Hospital) and work collaboratively with agency staff and key community partners to help the residents live in their homes and stay active in the community. 

NNORC Community Health Nurse Educator

Reports To: NNORC Director

Job Description: The Community Health Nurse Educator (CHNE) provides a variety of group and individual health screening and health education activities, consultation services, and provides healthcare/medical care coordination services.

The CHNE supports NNORC residents by providing person-centered health-based support and care coordination, provides residents with a variety of health education opportunities and engages in outreach activities designed to identify NNORC residents who have immediate and ongoing medical and health care needs. The CHNE also provides warm referrals for residents who have unmet social needs and supports NNORC social activities.

The CHNE provides individual healthcare consultation and helps residents manage chronic conditions, refers residents to appropriate medical providers to address acute episodes, and helps residents access and navigate the healthcare system. The CHNE engages residents in the assessment and care planning process, helps with arranging and coordinating services and provides follow-up and monitoring at least every two months.

Responsibilities and Duties:

  • Serve as a health education resource for residents by addressing health-related questions and concerns and respond to any unmet care needs by offering care coordination activities
  • Collaborate with health care providers to assist residents with the coordination of medical and healthcare services and supports
  • Develop and maintain professional relationships with key community partners
  • Complete intakes, assessments, consents, releases, respond to internal referrals, and document client contact
  • Coordinate with team members and participate in team and general staff meetings
  • Plan, direct, and coordinate health care assistance and management programs and activities
  • Monitor, evaluate, and redesign programs and activities and implement necessary improvements
  • Conduct telephone and in-person assessments and document resident needs sufficient to make appropriate referrals to health care providers/medical homes
  • Conduct regular blood pressure screenings clinics
  • Assist with new member outreach and engagement
  • Perform other appropriate duties as requested by Director of Neighborhood Naturally Occurring Retirement Community (NNORC) or CEO

** Direct care (i.e., poring meds, lifting, positioning, and other skilled service activities) to residents is not required or allowed. **

Qualifications:

  • Excellent oral, written, and telephone communication skills
  • Excellent organizational skills
  • Excellent communication and presentation and public speaking skills
  • Ability to work independently
  • Ability to work effectively within the medical community
  • Must be self-motivated and efficient, with a professional image and attitude
  • Knowledge of prevention and management strategies for a variety of common chronic health conditions (i.e., hypertension, diabetes, heart attack/stroke, Alzheimer’s disease, and other dementias)
  • Must have experience working with older adults aged 60 and over
  • Ability to collaborate well with team members, outside agencies and service providers
  • Proficient with Microsoft Office 365, Excel and PowerPoint
  • Valid New York State driver’s license with reliable, personal transportation
  • Ability to travel throughout the greater Capital Region, as needed.
