Aging Well

Our talented, experienced and compassionate staff collaborates with and empowers older adults in our community so they may live independently for as long as possible.

We are professional social workers, occupational therapists and care managers that have extensive expertise and skills in aspects of aging.

Together, we support older adults’ independence—health, safety and well-being, and fostering dignity and quality of life. We use a holistic, person-centered approach to offer older adults and their families support, guidance, referrals and direct services.

Our new Albany InPlace program is designed to improve safe navigation in the home through education, resources and modifications, if needed, to help those 60+ age safely at home. Learn more here.

Our geriatric social work care manager is specially train to help find resources to make older adults’ daily lives easier. They will work with you to form a long term care plan and find services you need. Learn more here.

The Albany Neighborhood Naturally Occurring Retirement Community (NNORC) is a unique three-square mile area in Albany that helps people aged 60 and over age in place. NNORC staff coordinates services and community resources that help seniors maintain their quality of life and independence. Learn more here.
