Jewish Family Services has expert case managers who collaborate with you to provide support, resources and referrals to meet your unique needs and situation.
Here are just some of the ways we can help:
  • DSS/Social Security assistance: Income issues or programs (MSP, Epic, Extra Help)
  • Connection with Insurance providers: Medicaid/Medicare or Medicare+ or NYConnects
  • Medical provider referrals
  • Mental health referrals
  • Medical transportation: MAS (Medicaid) or Community Caregivers (Medicare) or JFS Project 5
  • Aging in place: Accessibility equipment, insurance coverage
  • One-on-one assistance: Weekly activities (going through mail, paying bills)
  • Housing needs: applications and referrals to resources
