- DSS/Social Security assistance: Income issues or programs (MSP, Epic, Extra Help)
- Connection with Insurance providers: Medicaid/Medicare or Medicare+ or NYConnects
- Medical provider referrals
- Mental health referrals
- Medical transportation: MAS (Medicaid) or Community Caregivers (Medicare) or JFS Project 5
- Aging in place: Accessibility equipment, insurance coverage
- One-on-one assistance: Weekly activities (going through mail, paying bills)
- Housing needs: applications and referrals to resources
