Jewish Family Services of Northeastern New York (JFS) is seeking a Social Work Care Manager to support older adults across the Capital Region. This role is ideal for someone passionate about helping older adults maintain safety, dignity, and independence through compassionate, person-centered care.

Social Work Care Manager – Full Time (In‑Person)
Location: Capital Region (Albany, NY + surrounding areas)

Pay: $22.56–$24.61 per hour (commensurate with experience and credentials)
Schedule: Monday–Thursday, 37.5 hrs./week
Responsibilities

Manage a rotating caseload of approximately 40 clients with diverse needs.

Conduct client intakes, assessments, service plans, and follow-up

Provide person-centered care management to older adults in the Capital Region

Connect clients with community resources including food assistance, housing, healthcare advocacy, transportation, and mental health supports

Collaborate as part of an interdisciplinary team including clinicians, program managers, and community partners

Complete and track all referrals and applications to external agencies

Maintain accurate documentation and client records

Participate in Clinical Team Meetings and staff meetings

Contribute to grant reporting, outcomes tracking, and program evaluations

Support continuous improvement of processes and client services

Other duties as assigned by program leadership
Qualifications

MSW preferred; LMSW a plus

Minimum 2 years of care management or related experience (experience with older adults required)

Strong knowledge of community-based services

Excellent communication and organizational skills

Ability to work independently and as part of a team

Proficiency with Microsoft Office 365

Valid NYS driver’s license and reliable, personal transportation

Ability to travel throughout the Capital Region
